When planning your health decisions, Alberta Health Services utilizes something called "Goals of Care". These are advance directives for health care staff to read and follow, should you encounter the patient. These Goals of Care have been reviewed with the patient, the family, and the physician on the form.
The person owns the Green Sleeve - it is their property
The person should take their Green Sleeve with them to medical appointments (and bring it home!)
Not everyone needs a Goals of Care Designation order. People should discuss this with their doctor.
The Green Sleeve (with completed documents) should be kept on/near the fridge – if 911 is called, emergency responders know to look there.

Utilizing the Form During Emergency Calls
You must adhere to the Goals of Care on the sheet at all times. These are usually found on the fridge in the home, or on the back of their home entrance. If you arrive at a cardiac arrest call and someone in the home cannot show you the physical copy of the form, you must initiate full life-saving measures until it is found.
